This prescription originates from Jisheng Fang, modified from the Redness-Removing Powder of Xiao'er Yaozheng Zhijue (Unprocessed Rehmannia Root, Akebia Stem, Liquorice Root, Lophatherum Herb). The Redness-Removing Powder can clear the heart and cool the blood, purge the fire of the lower energizer small intestine, and is used to treat heart heat in children or heart heat transferring to the small intestine. Small Thistle Decoction is modified on this basis to become a formula that cools the blood and stops bleeding, promotes diuresis and relieves strangury, used to treat blood stranguria due to heat accumulation in the lower energizer.

In the formula, Small Thistle cools the blood and stops bleeding, serving as the main medicinal. It is assisted by lotus rhizome node and Typha to enhance the cooling of blood and stopping of bleeding, and also to resolve stasis, ensuring that bleeding stops without leaving stasis. Unprocessed Rehmannia Root and Gardenia clear heat, cool the blood, and stop bleeding, guiding the heat away to exhaust it. Akebia Stem, Bamboo Leaf, and Talc clear heat, promote diuresis and relieve strangury, gently draining to expel pathogenic heat through the lower orifices. Chinese Angelica is warm in nature, invigorating and nourishing the blood, preventing the excessive coldness of the other medicinals, and also guiding the blood along the meridians. All of the above serve as assistant medicinals. Liquorice Root alleviates urgency and pain, harmonizes the various medicinals, and serves as the guiding medicinal.

Modern medicine believes that this formula has hemostatic, diuretic, antipyretic, antibacterial, and anti-inflammatory effects. It is mainly used for: acute urinary tract infections, urinary tract stones, presenting with blood in the urine or hematuria, frequent urination, red and astringent heat pain, red tongue texture, thin yellow coating, and rapid pulse.
