The "Genealogy of the Qian Family" from Kuaiji states: It has been discussed that the postpartum condition is fundamentally due to blood deficiency, yin and yang exhaustion, and the impairment of qi. If heavily tonified, qi and blood may suddenly increase, but if improperly regulated, various pathogens can easily invade. The Four Substances Decoction avoids the cold nature of Peony Root, and benefits from the efficacy of ginger and peach. Qi should not be dissipated, and the method should include tonifying deficiency; food must support the spleen, not solely focus on digestion and elimination. Heat should not be treated with Scutellaria and Coptis, as it may cause stagnation of old impurities; cold should not be treated with cinnamon and aconite, as it may lead to new blood flow collapse. When the three yang meridians show many exterior pattern, it may seem appropriate to induce sweating, but using Ephedra would severely deplete yang. When the three yin meridians show severe interior syndromes, it may seem appropriate to purge, but using Chengqi Decoction would greatly deplete blood. Deafness and hypochondriac pain are due to kidney deficiency and lochia retention, so Bupleurum should not be used; delirious speech and excessive sweating are signs of weak yuan resembling evil, and should not be confused with stomach excess. Syncope is due to the decline of yang qi, making it difficult to distinguish between cold and heat, and only drastic tonification can restore yang. Convulsions are due to the deficiency of yin blood, and regardless of rigidity or flexibility, only nourishing the blood can moisten the collaterals. Tidal fever resembling malaria, if treated as malaria, will be prolonged; mental disorder resembling evil, if treated as evil, will quickly lead to exhaustion. All are fundamentally due to great deficiency and must be treated with drastic tonification. Excessive blood loss leading to dry stool, adding Cistanche to generation and transformation, is not something that moistening the intestines and harmonizing qi can resolve; suffering from excessive sweating and difficulty in urination, doubling the use of ginseng and astragalus in Liu Junzi Decoction, can promote the production of fluids and aid in urination. Adding ginseng and generation and transformation and taking frequently can save from postpartum danger; life-saving and longevity regulation must be emphasized before childbirth.

  1. In cases where the placenta does not descend, or there is cold blood and qi blockage, blood deficiency and weak qi, etc., taking Generation and Transformation Decoction two or three doses will cause it to descend, or taking one pill of Motherwort Pill with this decoction will also cause it to descend. This is because motherwort herb promotes blood circulation and nourishes blood, and its nature is to move without harming the body.
  2. This medicine is suitable for women both before and after childbirth.
  3. In cases of postpartum dizziness due to hemorrhage and deficiency dizziness, add fineleaf nepeta spike six to seven parts.
  4. In cases of postpartum qi deficiency and qi collapse, fatigue and weakness, add Ginseng, Astragalus Root.
  5. In cases of yang deficiency and reversal cold of limbs, add Aconite Lateral Root, Cassia Bark.
  6. For weak pulse and thirst, add Ophiopogon Tuber, Wuwei.
  7. For qi stagnation and phlegm, add Dried Tangerine Peel, Bamboo Juice.
  8. For blood deficiency and dry stool, add Hemp Seed, Bitter Apricot Seed, and Cistanche.
  9. For profuse sweating and insomnia, add Poria with Pine, Zao Ren, Astragalus Root; for profuse sweating in the upper body, add Ephedra Root; for profuse sweating in the lower body, add Fourstamen Stephania Root.
  10. For irritability and heat, add Moutan Bark, Chinese Wolfberry Root-bark.
  11. For lockjaw like wind, convulsions, and opisthotonos, add Schizonepeta, Saposhnikovia Root three to four parts each.
  12. For incomplete lochia, chills and fever, headache, and hypochondriac distension, there must be distending pain in the lower abdomen, add Carthamus, Moutan Bark, Cassia Bark three to four parts each, and one qian of Corydalis.
  13. For internal damage to diet, add hawthorn fruit, Dried Tangerine Peel, Villous Amomum Fruit, or Medicated Leaven, Germinated Barley.
  14. For external cold-damage disease and dampness, add Atractylodes Rhizome, White Atractylodes Rhizome.
  15. For blood stasis and food accumulation, dry stool in the stomach, and umbilical abdominal distension and fullness pain, add Rhubarb Rhizoma two qian.
  16. For postpartum bleeding that does not stop, or dark and black fistula disease water that is not red, or comes and goes, or is like water, or has clots, and continuous dripping, this is a sign of severe deficiency of qi and blood, and cold and cool medicines should not be mistakenly used. For floating pulse and collapse, add Aconite Lateral Root and other yang-tonifying medicines, otherwise there is no cure. Finger Citron Powder can be used alone with Chinese Angelica three qian, Sichuan Lovage Rhizome two qian, this is its modified formula.
