This prescription originally treats the syndrome of cold-damage disease where the pathogenic factor transmits to the Yangming or Shaoyin, transforming into heat and interacting with water, leading to the interlocking of water and heat, heat accumulation in the lower energizer, dysuria, or painful urination. The main mechanism of disease in the Polyporus Decoction syndrome involves both damage to fluid and water retention, creating a complex situation. When water qi stagnates and qi fails to distribute fluids, the already damaged yin becomes even more deficient; when body fluids cannot be distributed normally, the retained water accumulates further, resulting in an imbalance of dryness and dampness, commonly referred to as a disorder in fluid metabolism, manifesting as either local deficiency or excess. For instance, using only enriching yin methods exacerbates water retention, while using only diuretic methods further damages yin fluids. This not only fails to resolve the contradiction but also worsens the condition. Therefore, it is essential to combine enriching yin with diuresis. In the formula, Polyporus and Poria enter the kidney and bladder meridians. Polyporus is sweet, bland, and slightly bitter; the bitterness helps it descend directly to the Shaoyin, while the sweetness promotes the elimination of dampness. Poria eliminates dampness with bland medicinal properties and strengthens the spleen to control the source of dampness. Alisma promotes urination and clears heat. Talc is sweet, cold, and slippery, clearing heat and relieving stranguria. Donkey-hide Gelatin nourishes yin and moistens dryness, and also stops bleeding. The combination of these herbs clears heat and nourishes yin while promoting diuresis, ensuring that diuresis does not damage yin and enriching yin does not retain pathogens. This approach removes water qi, clears pathogenic heat, restores yin fluids, and alleviates all symptoms.
