The case treated here is toxic shock caused by Escherichia coli septicemia, with a high fever reaching 40.3°C, followed by a drop in blood pressure, cold limbs, and visual hallucinations, indicating a severe condition. This is due to a pre-existing deficiency of heart qi, leading to the reverse transmission of pathogenic heat to the pericardium. Symptoms include high fever, dry mouth, sore throat, a thin and rapid pulse, and a deep red tongue with scant coating, indicating that the pathogen has entered the nutrient level, damaging both qi and yin. The intense toxic heat is trapped internally, unable to disperse, resulting in cold limbs, a phenomenon where the deeper the heat, the more severe the coldness, resembling a yin state within extreme yang.
