The presence of sand-like particles in the urine, painful and difficult urination, or sudden interruption during urination, accompanied by a sense of urgency and pain in the urethra, acute lower abdominal pain, often sudden onset, severe colicky pain on one side of the waist and abdomen, sometimes extending to the external genitalia, and blood in the urine.

bubble_chart Modern Research

Urolithic stranguria is equivalent to urinary tract stones in Western medicine. Its disease cause is complex, with kidney stones being the most common. The clinical symptoms of kidney stones include lumbar pain, hematuria, pyuria, and in severe cases, the pain can lead to shock. Patients are often discovered after seeking medical attention due to severe colicky pain in the lumbar region; if kidney stones are suspected, a routine urine test is generally performed to check for microscopic cells, pus cells, casts, and crystals, as well as X-ray examinations of the kidneys and ureters; if necessary, intravenous pyelography or retrograde pyelography may be performed to observe and confirm the location of the stones. Treatment methods include symptomatic treatment such as intramuscular injections of pethidine and atropine for antispasmodic and analgesic effects, extracorporeal shock wave lithotripsy, and surgical stone removal.

In Taiwan, the prevalence of kidney stones is about 1%, with males being more affected than females, and the condition is most common between the ages of 20 and 40; the causes of kidney stones are generally due to various factors leading to the precipitation of crystalline substances in the urine within the kidneys, with more common factors including urethral obstruction, urinary tract infections, nutritional disorders, endocrine system diseases, and prolonged bed rest, among others. Many patients have no identifiable cause, which may be related to constitutional inheritance.

bubble_chart Pattern Analysis

  1. Dampness-heat Lingering for A Long Time︰Short and frequent urination, burning stabbing pain, dark yellow or reddish urine, accompanied by lower abdominal tension or alternating chills and fever, bitter taste in mouth, nausea and vomiting, or lumbago with tenderness, severe constipation, yellow and greasy tongue coating, full and rapid pulse. Overindulgence in rich and sweet foods leads to internal generation of dampness-heat, multiple abscess in the lower energizer, accumulation of dampness-heat, and prolonged scorching of urine, resulting in impurities forming sand or stones. It is advisable to use Eight-Ingredient Rectification Powder to clear heat and drain dampness, promoting urination.
  2. Dampness-heat Accumulation in the Lower Jiao︰Short and frequent urination, stabbing pain with burning sensation, dark yellow or reddish urine, accompanied by lower abdominal tension or alternating chills and fever, bitter taste in mouth, nausea and vomiting, or lumbago with tenderness, severe constipation, yellow and greasy tongue coating, and a rapid pulse. A preference for rich and sweet foods leads to internal generation of dampness-heat, multiple abscess in the lower energizer, and accumulation of dampness-heat, which scorches the urine. Over time, impurities in the urine form into sand or stones. It is advisable to use Eight-Ingredient Rectification Powder to clear heat and drain dampness, and promote urination.
  3. Liver Depression Transforming into Fire︰Liver depression transforming into fire shifts heat to the lower energizer, scorching urine to form stones. For those with liver depression and qi stagnation, Urinary Stone No. 1 can be selected.
This condition is often caused by external contraction of dampness-heat, or improper diet, leading to the accumulation of dampness-heat in the lower energizer, which condenses urine into stones.

bubble_chart Documentation

  1. Zhubing Yuanhou Lun‧Various Stranguria Syndromes: "Urolithic stranguria refers to the condition where stones are discharged during stranguria. The kidneys govern water, and when water congeals, it transforms into stones, hence the kidneys harbor sand and stones. Kidney deficiency is exploited by heat, which then leads to stranguria. The symptoms of this disease include pain in the urethra during urination, difficulty in passing urine, pain radiating to the lower abdomen, urgency in the bladder, and the passage of sand and stones through the urinary tract. In severe cases, the blockage and pain can cause fainting."
