Frenzy refers to a state of mental derangement, characterized by chaotic restlessness, irrational actions, cursing, singing, laughing, and noisy disturbances. It is commonly known as "martial madness" or "going crazy."

This condition was already discussed in a dedicated section on manic and depressive psychosis in the Lingshu. Later generations often compared manic and depressive psychosis. Although both manic and depressive psychosis fall under the category of mental derangement, they differ in terms of disease cause, mechanism of disease, and treatment. Therefore, this book will discuss "epilepsy" and "frenzy" in separate dedicated sections.

  1. Phlegm-Fire Harassing Upward︰The onset is relatively acute, with symptoms such as manic agitation, irritability, reckless behavior, incessant shouting and cursing, destroying objects and assaulting others, headache insomnia, flushed face, red eyes, severe constipation, red tongue texture, yellow and greasy tongue coating, and wiry, slippery, rapid pulse. This is caused by excessive heart and stomach fire, which scorches fluids into phlegm, leading to the binding of phlegm and fire that clouds the heart orifices. Abdominal distension and fullness make it impossible to lie down, accompanied by constipation, yellow urine, a yellow tongue coating, and a deep, rapid, and forceful pulse. This is due to pathogenic heat transmitting internally to Yangming; it is caused by heat accumulation in Yangming. Suwen. Yangming Maijie states: "The limbs are the foundation of all yang. When yang is exuberant, the limbs become replete, and when replete, one can climb high... When heat is excessive in the body, one discards clothes and desires to run... Yang exuberance causes people to speak wildly and curse, disregarding closeness or distance, and leads to anorexia. Anorexia thus causes aimless running." Suwen. Jue Lun states: "In the reversal of Yangming, there is manic behavior with a desire to run and shout, abdominal distension and fullness making it impossible to lie down, a red face with heat, delusional visions and speech." Therefore, the key diagnostic points are a red face without heat, abdominal distension and fullness making it impossible to lie down, wild speech and delusional visions, discarding clothes and desiring to run, disregarding closeness or distance, and anorexia. The treatment focuses on clearing and draining Yangming, with prescriptions such as Diaphragm-Cooling Powder or Major Purgative Decoction, among others.
  3. Liver-Gallbladder Stagnation Heat︰The mind is restless and agitated, failure of mind to keep to its abode, prone to irritability and anger, with abnormal speech, sometimes singing or laughing, palpitation due to fright and unease, chest and hypochondrium distending pain, a red tongue with yellow coating, and a wiry rapid pulse. This is caused by seven emotions internal damage, liver and gallbladder qi stagnation, qi depression transforming into fire, which disturbs mental activity upward. When the mind is disturbed, it becomes restless and agitated; when the spirit fails to remain within, speech becomes disordered, manifesting as singing, laughing, or talking incoherently; when the mind is unsettled, there may be fright or palpitations; liver and gallbladder qi stagnation leads to chest and hypochondrium distending pain. Therefore, the key diagnostic points are restless and agitated mind, incoherent speech, palpitation due to fright and unease, chest and hypochondrium distending pain, and a wiry rapid pulse. The treatment should focus on clearing and soothing the liver, and the recommended formulas are Gentian Liver-Draining Decoction combined with Peony and Gardenia Peripatetic Powder.
  4. Static Blood︰Chest tightness, restlessness, sometimes incessant talking, sometimes reticence, and in severe cases, continuous cursing and violent agitation, with lower abdominal distension and fullness, hardness, pain that refuses pressure, a red-purple tongue texture or ecchymosis, and a deep, strong pulse. This is caused by pathogenic heat entering the interior, leading to blood-heat binding and disturbing mental activity. The key diagnostic points include manic or violent behavior, hardness and pain in the lower abdomen that refuses pressure, and a red-purple tongue texture or ecchymosis. The treatment involves clearing heat and breaking blood stasis, with the selection of Peach Kernel Qi-Guiding Decoction or Resistant and Withstanding Decoction.

Mania belongs to the fu organs, with phlegm in the pericardium, thus it comes and goes; madness belongs to the zang organs, with phlegm gathering in the heart, thus it does not stop, this is the nature of manic and depressive psychosis. The affliction of mania, though rooted in the heart, mostly involves the liver; the affliction of madness, though rooted in the heart, also involves the stomach and kidney, this is the combined cause of manic and depressive psychosis."
