Spermaturia refers to the presence of semen mixed in the urine or the discharge of semen after urination.

Dampness-heat︰Short and reddish or turbid urine, difficulty in urination with a hot and astringent sensation or stabbing pain, often with rice-water-like or pasty turbid discharge dripping continuously from the urethral orifice after urination, itching and pain in the penis, or accompanied by seminal emission or spermatorrhea, along with bitter taste in the mouth, thirst, chest stuffiness, gastric stuffiness, difficulty in defecation, a red tongue with yellow and greasy coating, and a soggy and rapid pulse. This syndrome is an {|###|}interior heat excess pattern{|###|}, characterized by deteriorated semen mixed with urine, significantly turbid urine, and accompanying symptoms of {|###|}interior heat{|###|}. The treatment should focus on {|###|}clearing heat and draining dampness{|###|}, and the prescription of choice is Cheng's Rhizoma Dioscoreae Decoction for Clearing Turbid Urine. 2. Yin Deficiency︰Scanty and dark yellow urine with a burning sensation, turbid urine, or the presence of reddish turbid discharge from the urethral orifice after urination, often accompanied by nocturnal seminal emission, restless sleep, dizziness, dry throat at night, flushed cheeks and red lips, vexing heat in the chest, palms and soles, tidal fever and night sweats, a red tongue with thin coating, and a thready rapid pulse. The main causes include constitutional yin deficiency, unregulated sexual activity, Rebing damage to yin, etc., leading to frenetic stirring of ministerial fire, which disturbs the semen chamber (mingmen) and results in semen mixed with urine. This condition is attributed to kidney yin deficiency, with turbid discharge from the urethral orifice, often accompanied by symptoms such as restless sleep, vexing heat in the chest, palms and soles, and scanty dark yellow urine. Treatment involves nourishing yin and reducing fire, as well as securing the kidneys and astringing semen. The recommended formulas are Anemarrhena, Phellodendron, and Rehmannia Pill or Heaven, Human, and Earth Marrow-Retaining Pill, with the addition of astringent herbs as appropriate.
3. Kidney Failing to Store Essence︰Clear and profuse or frequent urination with seminal threads discharged after urination, painless urination, often accompanied by seminal emission or spermatorrhea, along with pale complexion, dizziness, blurred vision, tinnitus, deafness, aching pain in the lower back and waist, fear of cold, cold limbs, pale tongue with scant coating, and deep-thinny pulse. Due to prolonged illness impairing healthy qi or long-term seminal emission/spermatorrhea, yin impairment affects yang, leading to insecurity of kidney qi and failure of containment, resulting in semen mixing with urine and causing seminal threads in urine. Key points for differentiation: mainly kidney yang deficiency, often with seminal threads discharged after urination, no painful urination, frequently accompanied by pale complexion, aversion to cold, frequent urination, or clear and profuse urination. Treatment should focus on tonifying the kidney to constrain seminal emission, with prescriptions such as Great Dodder Seed Pill or Right-Restoring Pill, and modified Kidney Qi Pill from the Golden Cabinet.
