Heavy body refers to the symptom of heavy limbs, difficulty in movement, and difficulty in turning over.

This symptom is referred to as "body heaviness" or "heavy limbs" in ancient medical texts such as Neijing, "Cold-Damage Disease Treatise", and Zhubing Yuanhou Lun.

If the heavy and immobile limbs are caused by apoplexy, it falls under the category of "half-body paralysis" and is not within the scope of this section.

  1. Dampness Encumbering the Body Surface︰Heavy body pain, difficulty in turning over, fever and chills, headache with a distended sensation as if wrapped, chest tightness, anorexia, thin white greasy tongue coating, and soggy slow pulse. The disease cause is often due to wading in water or getting caught in the rain, exposure to mist or dew, or living in damp environments, where external dampness invades the skin and muscles. Dampness is sticky and heavy in nature, hence the heavy body; in more severe cases, the head feels distended as if wrapped, without skin edema, accompanied by symptoms of dampness obstruction such as chest pain, anorexia, and white greasy tongue coating. Treatment should aim to promote sweating and dispel dampness, with prescriptions such as Notopterygium Overcoming Dampness Decoction and others.
  2. Wind-Water Confrontation︰heavy body, facial edema, aversion to wind and cold, fever headache, sore limbs, cough, sore throat, scanty urine, thin white tongue coating, floating pulse. Due to wind evil invading the lung, the lung loses its function of dredging and regulating water passages, affecting bladder qi transformation. The evil is in the exterior, obstructing the meridians, hence heavy body and sore limbs. Key points of pattern identification: mild symptoms with facial edema, accompanied by aversion to cold with fever, sore throat, cough, and other symptoms of wind evil invading the lung. Treatment should focus on ventilating the lung and disinhibiting water, using modified Spleen-Qi Effusing plus Atractylodes Decoction.
  3. Waterlogging and Flooding︰heavy body without pain, lower limb edema that pits on pressure and does not rebound, complexion shallow yellow or pale and lusterless, anorexia and loose stool, mental fatigue and cold limbs, sore and heavy waist, scanty urine, pale and swollen tongue with white slippery coating, deep thin and slow pulse. This syndrome is caused by overstrain internal damage or prolonged illness without proper treatment, leading to spleen-kidney yang qi deficiency and internal exuberance of pudendal coldness. The spleen governs the four limbs and relies on yang qi for warming and transportation. Spleen-kidney yang deficiency results in water-dampness flooding, hence the heavy body. Key points for differentiation: heavy body without pain, limb fatigue and lack of strength, lower limb edema, accompanied by symptoms such as complexion shallow yellow or pale and lusterless, anorexia and loose stool, scanty urine, and sore waist. The difference between this and heavy body due to dampness adhering to the muscles and skin or heavy body due to wind-edema struggle is that the former belongs to interior pattern and deficiency pattern, while the latter both belong to exterior pattern and excess pattern. Therefore, the former heavy body does not involve body pain and manifests as heavy fatigue and lack of strength, signs of deficiency; the latter heavy body is accompanied by body pain and has exterior pattern symptoms such as headache, fever, aversion to wind, or aversion to cold. The two have clear distinctions. The treatment principle for edema due to yang deficiency is warming yang and excreting water, with formulas selected as Spleen-Strengthening Decoction and True Warrior Decoction with modifications.
Heavy body syndrome is often related to dampness, as stated in Zhangshi Yitong: "Heavy body is mostly attributed to dampness." Since dampness is a yin pathogen, it is heavy, turbid, and sticky in nature, and when it invades the body, it causes heaviness in the limbs. However, differentiation of patterns includes exterior and interior, as well as deficiency and excess. In exterior pattern, heavy body is often accompanied by cold, heat, and headache, while in interior pattern, it is often accompanied by symptoms of spleen and kidney deficiency, as well as internal retention of water-dampness. Exterior pattern should be treated with dispersing methods, while interior pattern should be treated with warming and moving methods.
