The pattern of dual deficiency of kidney yin and yang refers to a general term for a series of symptoms caused by insufficient primordial yang and deficient yin essence of the kidney, which cannot warm and nourish the zang-fu organs and meridians. The pattern of dual deficiency of kidney yin and yang is commonly seen in diseases such as "deficiency fatigue," "impotence," "seminal emission," "ischuria," "anuria and vomiting," and "edema."

This syndrome should be differentiated from the "kidney-yang deficiency pattern" and the "kidney-yin deficiency pattern."

In addition to the aforementioned clinical manifestations, the pattern of dual deficiency of kidney yin and yang exhibits distinct characteristics when it appears in different diseases, and should be carefully differentiated.

  • For example, in the case of ischuria, when the pattern of dual deficiency of kidney yin and yang is present, it is characterized by difficulty or dribbling urination, weak urination, soreness and weakness in the lower back and knees, tidal fever, and night sweats.   • In diseases with edema, the pattern of dual deficiency of kidney yin and yang is characterized by generalized edema, especially in the lower body, scanty urine, dry mouth, and a red tongue with little coating. This is due to kidney deficiency leading to dysfunction of urination and defecation, yang deficiency causing failure in qi transformation, and yin deficiency leading to fluids transforming into water, resulting in edema. Treatment should focus on tonifying the kidneys and promoting diuresis, using the formula True Warrior Decoction(from the "Treatise on Cold Damage Diseases") combined with Anemarrhena, Phelloendron and Rehmannia Pill(Yifangkao).
  • In cases of anuria and vomiting, the pattern of dual deficiency of kidney yin and yang is characterized by nausea, vomiting, scanty or absent urine, edema, a dull complexion, mental fatigue, and even mental confusion, dry mouth, a dark red tongue with a swollen body and white coating. This is due to severe deficiency of primordial yang, depletion of true yin, and retention of turbid pathogens. Treatment should focus on promoting yang to resolve turbidity, enriching yin, and promoting diuresis, using the formulas Golden Chamber Kidney Qi Pill(Jingui Yaolue) and Rhubarb and Aconite Decoction(Jingui Yaolue).
  • In cases of deficiency fatigue, the pattern of dual deficiency of kidney yin and yang is characterized by prolonged coldness with feverish sensations in the palms and soles, dry mouth but preference for hot drinks, dizziness, tinnitus, soreness and weakness in the lower back and knees, impotence, seminal emission, and infertility. This is often caused by extreme fatigue and prolonged deficiency that fails to recover, leading to dual deficiency of kidney yin and yang. Treatment should focus on dual supplementation of yin and yang, using the formulas Right-Restoring Pill and Left-Restoring Pill(Jingyue Quanshu). Clinically, differentiation should be made based on the specific disease.

    The pattern of dual deficiency of kidney yin and yang often appears in the advanced stage of diseases, with severe decline of healthy qi and damage to both yin and yang, representing the final pathological transformation. In the process of disease mechanism evolution, it ultimately leads to the exhaustion of kidney qi and the separation of yin and yang, resulting in a critical condition. Treatment should follow the principle of "treating yin for yang, treating yang for yin," urgently rescuing the endangered yin essence and yang qi.

It can also result from prolonged and untreated kidney-yin deficiency pattern, where yin impairment affects yang, evolving into a syndrome of deficiency of both yin and yang. Yang deficiency manifests as fear of cold and curling up; yin deficiency manifests as a feverish feeling in the palms and soles, dry mouth with a desire to drink. The kidneys govern urination and defecation, so true essence deficiency leads to clear and copious urine or dribbling after urination, impotence, and seminal emission in men, and infertility in women. The chi pulse corresponds to the kidneys, so kidney deficiency results in a deep and thin chi pulse. Pure kidney-yang deficiency pattern is less severe than the pattern of dual deficiency of kidney yin and yang, clinically presenting with fear of cold, pale complexion, sore and cold lower back and knees, edema, and clear and copious urine, primarily reflecting yang decline and yin exuberance. This can be used for differentiation.

    2. Kidney-yin deficiency pattern and pattern of dual deficiency of kidney yin and yang: Kidney yin deficiency is often caused by excessive indulgence damaging essence, or Rebing late stage [third stage], scorching kidney yin, or chronic illness affecting the kidneys. The mechanism of disease for the pattern of dual deficiency of kidney yin and yang can develop from prolonged kidney-yin deficiency pattern, and the condition is more severe than pure kidney-yin deficiency pattern. Clinically, the former primarily manifests as vexing heat in the chest, palms, and soles, insomnia, night sweats, dry mouth and throat, sore and weak lower back and knees, nocturnal emission, a red tongue, and a thin, rapid pulse, reflecting yin deficiency with effulgent fire. The latter additionally includes fear of cold, curling up, edema particularly in the lower body, impotence, and cold leukorrhea in women. The two conditions differ in severity and depth.
