Latent glomerulonephritis, also known as latent glomerular disease, was previously referred to as asymptomatic proteinuria and/or hematuria. It is characterized by mild to moderate (grade II) proteinuria and/or hematuria, with a prolonged course, and typically does not present with edema, hypertension, or renal impairment.

1. Medical History and Symptoms

Most patients have no clinical symptoms and only discover abnormal urine tests during physical examinations. A few may present with gross hematuria but without edema or hypertension. Some may experience lumbar soreness, which worsens after exertion or infection but alleviates after treatment.

2. Physical Examination Findings

Most physical examinations show no positive signs.

3. Auxiliary Examinations

The 24-hour urine protein quantification is mostly below 2g, primarily albumin. Microscopic hematuria is present in the majority of cases, with red blood cells showing polymorphism and diversity. Renal function is normal, rheumatoid factor and antinuclear antibodies are negative, and complement levels are normal. Isotope renal scans, renal B-ultrasound, and intravenous pyelography are all normal.

4. Differential Diagnosis

It should be differentiated from functional or orthostatic proteinuria, IgA nephropathy, subcutaneous nodules of the urinary system, renal tumors, etc.

1. Avoid common cold and excessive fatigue in daily life. For those with recurrent common cold, transfer factor, thymosin, or tonsillectomy may be considered. Protein intake should not be too high, and nephrotoxic drugs are prohibited. If there are recurrent infection foci, they should be removed.

2. Root Leaf or Flower of Common Threewingnut glycosides or low-dose ACEI can be used, along with long-term vitamin C intake of 1.0g/d.

3. Chinese medicinals such as Six-Ingredient Rehmannia Pill, Anemarrhena, Phelloendron and Rehmannia Pill, and Chinese Caterpillar Fungus preparations have certain therapeutic effects.
