Normal semen is milky white or milky yellow. If blood is mixed in the semen, it turns red, brownish-red (coffee bean color), or contains blood streaks, which is called hemospermia.

1. Inflammation of the seminal vesicles and prostate, subcutaneous node, stones, tumors, or prostatic hyperplasia;
2. Trauma, such as cycling, excessive sexual activity, or prolonged separation of couples;
3. Secondary to other systemic diseases, such as schistosomiasis, liver cirrhosis, or bleeding disorders (leukemia, thrombocytopenia).
Clinically, hemospermia is mostly caused by seminal vesiculitis.

The main symptoms include decreased libido, premature ejaculation, impotence, and painful ejaculation. If caused by seminal vesiculitis, sexual arousal may increase, accompanied by grade I perineal or lower abdominal discomfort (severe cases may present with abdominal pain), frequent urination, urgency, dysuria, or post-urination dribbling. Other symptoms related to the underlying disease may also occur. Married men with hemospermia often go undetected early if symptoms are mild. Minor bleeding is easily overlooked, while slightly more noticeable bleeding may be mistakenly attributed to the female partner. Therefore, those with the above symptoms should observe the color of their semen. If hemospermia is suspected, a hospital examination is necessary. In cases of seminal vesiculitis, a rectal exam may reveal significant tenderness above the prostate. Semen culture may also be required to identify pathogens and guide the selection of anti-inflammatory medications for complete recovery.
The principle is anti-inflammatory and etiological treatment, with hemostatic medication administered when necessary. During the illness, alcohol and spicy, irritating foods should be avoided, and warm sitz baths should be taken once or twice daily. Young and middle-aged adults should moderate sexual activity, avoiding excessive frequency, but maintaining it once a week. Most scholars believe that regular ejaculation aids recovery and reduces recurrence rates.
