For kidney yang deficiency. Symptoms include mental fatigue, abdominal pain, cold reversal of hands and feet, pale tongue with white coating, and deep, thin pulse. Or for conditions of exuberant yin repelling yang, true cold with false heat.

The kidney is the root of the yang qi of the whole body. Insufficient kidney yang cannot warm, leading to a deficiency in the generation and transformation of qi and blood, resulting in a lack of nourishment, hence manifesting as timidity and mental fatigue. The waist is the residence of the kidney, and kidney deficiency leads to soreness in the waist. Kidney yang cannot warm and assist spleen yang, leading to yang deficiency cold in the middle, hence manifesting as cold pain in the abdomen. Yang deficiency is unable to reach the limbs, hence reversal cold of hands and feet. A pale tongue and deep, thin pulse are all signs of yang deficiency. Exuberant yin repelling yang, true cold with false heat, means there is true cold inside and false heat outside. This is due to the decline of kidney yang, with excessive pudendal coldness inside, and yang having no place to rely on, being repelled outside. Clinically, it manifests as excessive pudendal coldness inside, but with external signs of floating heat, thirst, restless hands and feet, and a large pulse, which are symptoms of false heat. However, although the patient's body is hot, they prefer to cover themselves with clothes and blankets; although thirsty, they drink little water, preferring hot drinks or rinsing their mouth without swallowing; although their hands and feet are restless, their mind is clear; although the pulse is large, it is weak upon pressing. These are signs of true yang deficiency decline, commonly seen in patients with organ failure. Therefore, the formula primarily uses Prepared Rehmannia Root, sweet and warm, to nourish the kidney and replenish essence; with Cornus and Barbary Wolfberry Fruit to nourish liver blood, combined with the main herb to greatly nourish the liver and kidney; Chinese Yam, Liquorice Root to tonify the middle and benefit the spleen, aiding the source of generation and transformation; Eucommia Bark to strengthen the waist and knees, and fortify the bones and tendons; Cassia Bark, Aconite Lateral Root to warm the interior and dissipate cold, combined with the main herb to support yang and transform qi. Combining all these herbs can achieve "Only when yin is at peace and yang is compact can essence-spirit be normal."
