Mr. Zhang, male, 74 years old. Initial consultation on November 9, 1985. After undergoing a "prostatectomy" at the end of July of the same year, the patient experienced external-contraction fever. After treatment with both Chinese and Western medicine, the fever subsided, but he developed aching pain and weakness in both lower limbs, requiring assistance to walk. He also experienced stiffness and pain in both sides of the neck. After more than two months of treatment with Chinese and Western medicine at another hospital, the symptoms in the lower limbs gradually improved, but the neck symptoms worsened. Examination revealed a body temperature of 35.5°C, a pained expression, clear speech, and a leftward tilt of the head and neck. Turning the neck to the left caused pain. <60°，右側<45°，俯仰均受限。症見身體瘦，頭項左傾，兩側頸項和後枕部僵硬麻木，牽強疼痛，轉側時疼痛益劇，頭似不在脖子上，二便自調。舌質淡紅，苔薄白，脈細弦。觀前醫處方多為羌防一類祛風濕止痛或夾通絡養血之品，然患者頸項諸症實屬仲聖所謂 "強几几" 也，其太陽證已躍然眼前，遂處以《傷寒論》葛根湯原方：
Pueraria Root 40g, raw Ephedra 10g, Cinnamon Twig 10g, red Peony Root 30g each, raw Liquorice Root 10g, Fresh Ginger Rhizome 3g, Chinese Date 12 pieces
The patient was advised to cover lightly to induce slight sweating after taking the medicine. After two doses, the patient's head and neck returned to their normal position, and he felt more energetic. He reported slight sweating after taking the medicine that day, with a faint sensation of heat in the neck area, and a significant reduction in symptoms. The neck felt much looser, as if a heavy burden had been lifted. The next day, after taking the medicine without sweating, the neck symptoms disappeared completely, and he could turn his neck freely, with only slight dizziness. As the condition was cured, no further prescription was given. A month later, he was seen in the outpatient clinic and reported that everything was fine.
