Lin Xiusong, around thirty years old, had been ill for forty days. Initially, he experienced chills, fever, extreme thirst, and constipation. Recently, he only removes his blanket when it's hot, has bowel movements every three to four days with difficulty, and passes stool in small amounts. His urine is normal, but he has no appetite and feels no bloating. Upon waking, his lips, tongue, and throat remain dry, and he drinks half a cup of water. His tongue is slightly red with a white, slippery coating, and he frequently spits out sticky phlegm, which he needs to cough up to expel. Previously, he vomited continuously after consuming water or food, but now he only vomits after eating porridge, expelling only saliva-like fluid without any solid food. He can slowly consume soup and tea. His hearing is still impaired, and his pulse is slightly wiry with a slippery quality upon deep pressure (though not extremely strong, it does not show signs of deficiency or weakness). Despite being ill for over a month, his spirit is not significantly weakened. Reviewing his previous prescriptions, they were all from the disciple of Chen Xiuyuan, surnamed Zheng, who used classic formulas from Zhang Zhongjing (such as Ophiopogon Decoction, Pinellia Heart-Draining Decoction, Poria and Alisma Decoction, Golden Cabinet Tangerine Peel and Bamboo Shavings Decoction, Bamboo Leaf and Gypsum Decoction, etc., all of which had been fully administered). Later, Major Bupleurum Decoction was considered but not administered, and Yan Bingfu used Five Ingredients with Poria Powder. I commented that since there was no excessive thirst and the pulse was not deficient, it was not a Bamboo Leaf and Gypsum Decoction syndrome; there was no epigastric stuffiness or diarrhea, so it was not a Pinellia Heart-Draining Decoction syndrome; there was no vomiting or throat discomfort, nor abnormal rising of qi due to malpractice of heat therapy, so it was not an Ophiopogon Decoction syndrome; with normal urination and movement in the epigastrium, only vomiting of food, it was not a Poria and Alisma Decoction syndrome; the pulse was not deficient, so it was not a Tangerine Peel and Bamboo Shavings Decoction syndrome; there were no chills or fever, and the abdomen was soft, so it was not a Major or Minor Bupleurum Decoction syndrome; vomiting after eating, not after drinking water, also ruled out Five Ingredients with Poria Powder for water counterflow syndrome. I prescribed Dried Ginger, Coptis, and Ginseng Decoction, using Glehnia to treat heat obstruction, and the patient recovered after one dose.
