Che, male, 74 years old, in early April 1975, felt wind-cold and general discomfort. Female, body temperature 39°C, diagnosed with common cold and high fever, was injected with gentamicin and given Western medicine, but the high fever persisted. The tendency of the disease was critical, so Fan Lao was invited for an emergency home visit.

The patient had intermittent unconsciousness, faint pulse, and had been suffering from high fever for three days. Despite the abnormal body heat, the patient still felt cold in the middle of the chest even when heavily covered. The patient had not eaten, had constipation, flushed cheeks, a pale and moist tongue, and thick, greasy, black coating. Over seventy years old, with excessive pudendal coldness, there was a fear of imminent death. Although there was a Taiyang (EX-HN5) exterior pattern, the priority was to save the interior, so the Vessel-Unblocking Cold-Extremities Decoction was urgently administered.

The patient had high fever, unconsciousness, flushed face, black coating, and constipation, resembling signs of yang heat. Despite the high fever, the patient desired heavy covers. The body was hot and the face flushed, but the limbs were cold. There was constipation, but no abdominal pain. The tongue coating was black, thick, and greasy, but the tongue was moist. High fever and unconsciousness without delirium, and the pulse was deep and faint. Considering the patient's advanced age, weak constitution, and declining qi, it was a clear case of shaoyin solitary yang flying away, with vitality about to leave, and imminent death. Therefore, the Vessel-Unblocking Cold-Extremities Decoction with scallion was urgently administered to chase the dissipating and nearly extinguished yang.

Second consultation: After taking two doses of the above prescription, the fever subsided, the black coating significantly reduced, the yang returned, and the yin haze began to dissipate. The yin-yang rejection pattern was resolved. However, headache, body pain, and exterior pattern remained; kidney yang deficiency was severe, unable to transform qi, so constipation persisted. Ephedra, Aconite, and Liquorice Decoction was used to expel cold and consolidate yang, with scallion to generate shaoyang and expel pathogens from the exterior. Prescription:

Third consultation: After taking four doses of the above prescription, the dizziness was gone, bowel movements were normal, the black coating completely disappeared, but body pain remained. Although yang returned and the exterior was resolved, the tongue was still pale and the limbs cold. Pudendal coldness was severe, showing signs of yang deficiency body pain. It was appropriate to warm and raise the original yang to expel cold, using the Vessel-Unblocking Cold-Extremities Decoction with Asarum.

Fourth examination: After taking two doses, all remaining symptoms were resolved. After recovering from the serious illness, true yang deficiency was severe, so Middle-Regulating Decoction with modifications was used for regulation. Prescription: Tangshen 15g, Stir-fried White Atractylodes Rhizome 10g, Prepared Liquorice Root 10g, Dried Ginger 15g, Prepared Aconite 30g, Poria 12g. On July 18, 1979, a follow-up visit found the patient, now 79 years old, in good health since recovery.
