Huang, female, 80 years old, diagnosed on May 6, 1985. The patient had abdominal pain for 3 days, and after admission, she developed convulsions in her limbs and was unconscious. Despite treatment with various Western medicines, her condition remained critical. The next evening, her family carried her home to await death. This seasonal disease caused her convulsions to become more frequent, and she was in a coma, not recognizing anyone. Upon recommendation, she was brought to me for treatment.

The patient lay facing the wall, moving slightly upon hearing sounds, mumbling without sound; at times, she would flail her limbs, convulsing, with her eyes open and staring, glowing brightly; at other times, her eyes were unclear, like those of an eagle, with rough breathing and slight panting; her lips were red and dry, her tongue was red with thick, dry white coating, and her mouth had a foul, unbearable odor. There was distension and fullness in the upper abdomen, painful upon pressure. Upon questioning the family, it was learned that she had not had a bowel movement for seven days. All signs of the yangming fu-organ pattern were present. Although she was old and thin, it was indeed a case requiring drastic purgation, and Major Purgative Decoction was prescribed:
Raw Rhubarb Rhizoma 10g (to be decocted later), Mirabilite 30g (to be decocted later), Liquorice Root 20g, thickly prepared, 10g Immature Orange Fruit 10g
The above five ingredients were boiled in water to yield one bowl, to be taken warm in two doses. After taking the medicine, she indeed passed a large amount of dry stool, and by the afternoon, the patient regained consciousness. After several days of further treatment, she was declared cured.
