Jiang, male, 34 years old. Diagnosed on July 15, 1975. At the end of 1974, palpitations occurred frequently, with stuffy pain in the left chest, fixed pain points, shortness of breath as if discontinuous, mental fatigue and lack of strength, inability to speak much, sometimes dry mouth with a bitter taste, yellow urine, dysphoria, poor sleep, dark red tongue texture with ecchymosis on the edges, slightly yellow coating, wiry pulse with occasional knots, rapid beats, and missed beats (occasional bigeminy or trigeminy). Long-term treatment showed little effect. The syndrome is attributed to deficiency of heart qi and blood, and obstruction of the collaterals. The treatment should focus on tonifying and nourishing qi and blood, and dredging the collaterals. The prescription used is Prepared Liquorice Decoction:
Prepared Liquorice Root 30g, Unprocessed Rehmannia Root 60g, Ophiopogon Tuber 30g, Donkey-hide Gelatin 6g, Hemp Seed 9g, Tangshen 9g, Cinnamon Twig 4.5g, Fresh Ginger Rhizome 3 slices, Chinese date 10 pieces, white wine 2 spoons (take infused)
. After taking 5 doses, the patient felt significant improvement, with a great reduction in premature beats, improvement in palpitations, better sleep, and appetite. The wiry pulse showed signs of receding. However, the patient still felt shortness of breath and lack of strength, unable to engage in physical labor. The prescription was adjusted by increasing Tangshen to 30g and adding 3g of red ginseng. After taking more than 20 doses, the premature beats basically disappeared, strength gradually increased, the patient could speak more, the ecchymosis on the tongue edges subsided, and the pulse manifestation became normal. Finally, the prescription was adjusted again and 15 more doses were taken, with premature beats rarely occurring. The prescription was adjusted again for follow-up treatment.
