title | Medical Cases of Ha Litian |
Zhang, female, 24 years old, unmarried, first consultation on September 13, 1972. Generally healthy, but about ten days ago, after labor, she sweated and was exposed to wind. At night, she felt aching pain all over her body, a fear of cold, and her menstruation, which was ongoing, suddenly stopped. She drank a bowl of ginger sugar water and covered herself to induce sweating. After sweating, the body pain decreased, but the fever remained high, with an underarm temperature of 39.8°C. A local health center diagnosed her with a common cold and administered an injection of antondine and oral antipyretic analgesics. Although her temperature dropped afterward, she still experienced intermittent chills and fever in the afternoon, with an underarm temperature of 37.4°C. She also felt irritable, had hallucinations and auditory hallucinations, often hearing teasing and abusive language, leading to outbursts of anger or uncontrollable laughter, worse at night, and restless sleep. A hospital diagnosed her with "neurasthenia" and prescribed chlorpromazine and other sedatives, which were ineffective.
Upon inquiry, she had abdominal distension and fullness, pain that she did not want to press, a bitter taste in the mouth, nausea, and no appetite. Her tongue was red with thin yellow coating, and her pulse was wiry, thin, and rapid. The diagnosis was heat entering the uterus. The treatment plan was to clear heat and outthrust pathogenic factors, harmonize the liver and gallbladder, cool blood and resolve stasis, and tranquilize the spirit. Prescription:
