Gao, male, 28 years old, first consultation on August 15, 1961. The patient initially had fever and chills, followed by swelling and pain in the testicles, extending to the lower abdomen, and despite medical treatment, there was no improvement. He was bedridden for five to six days, with a confused state of mind, fever, delirious speech, restless hands and feet, and occasional mental instability, even biting others. His stool was not constipated, and urination was normal. He had a dry mouth but no desire to drink. The lower abdomen was hard and full, tender to touch, with a thin yellow tongue coating, and a deep, wiry pulse. The "Treatise on Cold Damage Diseases" states: "If a person becomes manic, it is due to heat in the lower jiao, the lower abdomen should be hard and full, and if urination is normal, the condition will be cured by the discharge of blood." Currently, urination is normal, the lower abdomen is hard and painful, tender to touch, with delirious speech and manic behavior. The syndrome is heat accumulation in the bladder. The method of breaking and expelling blood stasis is proposed, following the idea of Peach Kernel Purgative Decoction with modifications:
Peach Kernel 9g, Tail of Chinese Angelica 9g, Wine-processed Rhubarb 9g (added later), Red Peony Root 9g, Mirabilite 9g (dissolved), Stir-fried Pangolin Scales 6g, Bat Feces 6g, Immature Orange Fruit 9g, Magnolia Bark 9g, Moutan Bark 9g, Carthamus 6g, Liquorice Root 6g
, decocted with water, divided into two doses, one dose per day. After taking 3 doses of the above medication, a large amount of black stool was discharged, sticky like lacquer, the fever subsided, the mind cleared, the pain stopped, the swelling subsided, and the condition was cured.
