Note: There are two types of reversal syndromes, yin and yang. In cold reversal, the body is cool and not thirsty, with a slow, thin, and faint pulse. In heat reversal, the yang heat is extreme and leads to reversal, with symptoms such as delirium despite the reversal, intermittent warmth of the body, and a rapid pulse. If the heat reversal is extremely severe, leading to a cold body and a faint pulse on the verge of disappearing, it indicates extreme heat and impending death. Immediate purgation with Da Cheng Qi Decoction can cure the reversal, as cold medicine can paradoxically revive the reversal and warm the body. If hot medicine is used to support the yang, the yin qi will suddenly be exhausted, and the yang will also be exhausted, leading to death. If the yin has already been exhausted and the yang is about to be exhausted at this time, and the decoction is delayed, both yin and yang will be depleted, resulting in death. For cold reversal, Bai Tong Decoction for reversing cold limbs should be administered urgently; delay will render it ineffective.

Appendix: Onion Fumigation and Moxibustion Method, specifically for treating yin toxin reversal cold of hands and feet, sudden abdominal pain and collapse. After taking White Unblocking Decoction or Cold-Extremities Decoction, use a large handful of onions, tie them with a rope, cut off both ends, leaving about an inch of white part. Heat it over a fire and place it on the navel. First, fill the navel with half a portion of musk, then place the onion cake and use a hot iron to fumigate, allowing the heat to enter the abdomen through the navel. For severe pain, fumigate two or three cakes in a row. When the body becomes warm and sweating occurs, the condition will be cured; otherwise, it is incurable. Alternatively, use moxibustion on the Guan Yuan Qi Hai (one and a half inches below the navel, known as Qi Hai, two inches as Dantian, and three inches as Guan Yuan) for twenty to thirty cones each. Attack both internally and externally to ensure that within a short time, the yin disperses and the yang returns, achieving a cure through sweating. (Onions can unblock the middle, moxa is warm and hot, musk can resuscitate, and with the aid of fire, it has the effect of restoring yang.) Some say that using strong vinegar mixed with bran, stir-fried until cooked, placed in a bag, and steamed for fumigation is even more effective than the previous method.
