title | Medical Case of Zhang Shengquan |
A young child, 3 years old, has had poor appetite for the past three to four days, accompanied by abdominal pain, abdominal distension and fullness, nausea, vomiting, thirst for cold drinks, hot breath from the mouth and nose, dry stool, short and yellow urine, yellow and greasy tongue coating, slippery and rapid pulse, and purple and stagnant finger veins. The syndrome is attributed to food injury (dyspepsia) abdominal pain, treated with laxation to remove stagnation, using Minor Purgative Decoction:
Rhubarb Rhizoma 5g, Immature Orange Fruit 3.5g, Magnolia Bark 3.5g
decocted with water, to be taken until bowel movement occurs, then stop. (Zhang Xiuyun's medical case)‧(2) Ji, male, 22 years old. A month ago, the patient fell from a height of 6 meters, immediately experiencing severe pain in the lower back and inability to move both lower limbs. The next day, constipation occurred. X-ray showed a wedge-shaped change in the first lumbar vertebra, with about 2/3 compression of the vertebral body and backward convex deformity. He was admitted to a hospital for treatment. After admission, he relied on enemas and catheterization for bowel and bladder movements. After more than a month of treatment, there was no improvement.
Examination revealed a backward convex deformity in the first lumbar vertebra, obvious tenderness, right lower limb muscle strength at grade I, right ankle drop, left lower limb muscle strength at grade 0, and absence of anal, cremasteric, knee, and Achilles tendon reflexes. Multiple hard masses were palpable in the lower abdomen, constipation, red tongue texture, slightly yellow and thick greasy coating, deep, thin, and forceful pulse. The syndrome is attributed to obstruction of bowel qi, internal disturbance of turbid qi, failure of clear yang to rise, and malnutrition of the limbs. Minor Purgative Decoction was urgently administered to unblock bowel qi, allowing turbid yin to descend and clear yang to rise, warming and nourishing the limbs. The prescription:
Rhubarb Rhizoma (to be decocted later) 25g, Magnolia Bark 15g, Immature Orange Fruit 10g
Three hours after taking the medicine, the patient felt abdominal gas movement. The next day, the above prescription was taken again with the addition of Plantain Seed 10g and Akebia Stem 15g
